July 1, 2025 All-Payer OASIS Rollout: What Your Agency Needs to Know (Without the Confusion)

There’s been a lot of buzz—and confusion—about the July 1, 2025, All-Payer OASIS rollout. Many agencies are unsure if it applies to them, especially those providing non-skilled services.
Let’s clear things up.
First, What Is the All-Payer OASIS Rollout?
Starting July 1, 2025, all Medicare-certified Home Health Agencies (HHAs) must collect and submit OASIS data on all adult skilled patients, regardless of the payer source.
That means whether the patient is covered by:
- Medicare Advantage
- Medicaid
- Commercial insurance
- Self-pay
- Charity care
OASIS will be required if they’re receiving skilled care from a Medicare-certified HHA.
Does This Apply to Non-Skilled Home Care Agencies?
No.
If your agency only provides non-skilled services—like personal care, companionship, or housekeeping—and you are not Medicare-certified, this rule does not apply to you.
You do not need to start collecting or submitting OASIS.
Who Is Exempt from OASIS (Even After July 1)?
Even if you’re a Medicare-certified HHA, the following patients remain exempt:
- Patients under 18 years old
- Maternity patients (receiving care for pregnancy or postpartum conditions)
- Patients receiving only non-skilled services (like personal care or homemaking)
- Patients seen only by a social worker or home health aide
- Patients who were referred but never admitted
What Should Medicare-Certified Agencies Do?
If you’re a Medicare-certified home health agency, make sure your clinical and intake teams know:
- Starting July 1, OASIS applies to all skilled adult patients, regardless of insurance.
- Train your staff to identify who is exempt (see list above).
- Double-check that your documentation and systems are ready to support OASIS data collection across all payer types.
